• After you have downloaded and printed the forms, complete the required information.

Now what? Our New Patient Center is equipped to help you get things started, making the new-patient intake process as seamless as possible.

At Lake Travis Eye and Laser Center, we are professional, yet capable of upholding the flexibility you need to meet your busy lifestyle needs. This not only comes across in our office, but also comes across online. Our online patient center streamlines the process, making it easy for you to find payment information and complete all the necessary paperwork prior to your first visit.

All online forms are designed to give Dr. Rhodes and Dr. Wenn a glimpse at your previous and current medical history, as well as help the eye doctor understand your questions, concerns, and goals regarding your vision and eye health. With this information, Dr. Rhodes, Dr. Wenn and our staff are able to better understand your needs and ensure you get the quality eye care you deserve.

As part of our New Patient Center, we have compiled the forms you will need to fill out prior to your first visit with us. Unlike some offices that bombard you with paperwork as soon as you sign in, we give you access to forms so that you can fill them out at your own pace and in the comfort of your home or office.

We want your transition to our office to be smooth. Your time here should be spent relaxing and getting to know the staff that will be caring for you, not attending to questions and shuffling through papers.
